Abstract: 

Background and Aim: Regarding the importance and prevalence of lichen planus among patients and rarity of Oro- Vulval-Vaginal and Peno-Gingival syndromes, that need more attention for diagnosis because of simultaneous oral and genital involvement, we are going to review the symptoms and early diagnosis of these patients. The aim of this article is to report two patients with Oro-Vulval-Vaginal syndrome and two patients with Peno- Gingival syndrome with a short review on these syndromes and their treatments. Case Report: This study is a case report four patients with these syndromes who attended oral medicine department. For each patient a complete document consisting of demographic information, medical, dental, familial and social history was prepared and a photograph was taken from the oral lesions at the first visit and for definite diagnosis of genital lesions consultation with Dermatologist and OB &GYN Surgeon was done. After biopsy and definite diagnosis simultaneous treatment for oral and genital lesions started.Regarding the severity of lesions and acceptable response to the primary treatment, type and dose of the drug was changed and finally the patients were placed in follow up phase.Conclusion: Regarding the outcome of neglecting Lichen Planus in other organs and delay in accurate diagnosis and lack of receiving enough and on time treatment, performing an exact examination and taking a complete history with attention to other organ's symptoms is of great importance. More over malignant changes in even asymptomatic oral and genital lesions is probable.

Abstract: 

Background: Evidence suggests the role of the Vaginal microbiome and microenvironment in the immunity state. The human papillomavirus (HPV) infection is widely dependent on the healthy Vaginal microenvironment. Hence, this study aimed to investigate the role of the Vaginal microenvironment in the rate of high-risk HPV (hr-HPV) infection. Materials and Methods: This cross-sectional study was performed on 512 women with hr-HPV positive (n=212) or negative (n=300) infection. The Vaginal samples of women were examined regarding yeas and Gardnerella Vaginalis infection. Also, Lactobacillus acidophilus, pH, and enzyme activity (such as catalase, proline aminopeptidase, and leucocyte esterase) were compared between the two groups. Also, the histopathological study was performed on the Vaginal samples. Results: The higher rate of yeast and G. Vaginalis infections as well as decreased L. acidophilus, were significantly observed in women with hr-HPV positive infection (P<0. 001). Also, histopathological findings indicated that cervical intraepithelial neoplasia grade I-III and cervical cancer lesions were markedly higher in hr-HPV positive group compared with control women. Conclusion: The hr-HPV infection was markedly correlated to Vaginal microenvironments, and it could a risk factor for the elevation of the rate of high-grade cervical lesions.

Abstract: 

Background: C. trachomatis is one of the most important agents of sexually transmitted diseases (STD). Untreated cases of Vaginal infection with this bacterium may end up with infertility, scaring and ectopic pregnancy. In most cases infection with C. trachomatis is silent (70-80%) but when overt infection occurs the symptoms include: cervicitis, urethritis, and endometritis.Since prevalence of C. trachomatis was not studied in our city, this project was performed to evaluate the role of this bacterial agent in vaginosis.Material and Methods: Two hundred and two specimens from Vaginal discharge of out-patients referring to gynecology ward of Ahvaz hospitals were collected and tested for C. trachomatis by PCR.Results: Out of 202 specimens, 33 (16.3%) were positive for C. trachomatis.Conclusion: The findings of this study show a high prevalence of C. trachomatis which indicates that special attention must be paid to detection and management of this infection.

Abstract: 

Introduction: Bacterial vaginosis (BV) is the most common vaginitis in childbearing women. Despite the availability of various treatments, recurrence rates are high. This study investigated the effects and side effects of vitamin C Vaginal tablets in patients with Gardnerella Vaginalis infection based on Amsel criteria and the culture. Methods: A double-blind, randomized clinical trial was conducted on 48 non-pregnant women aged 15-40 years referred to Alawi Hospital in Ardabil Province, Iran. After the diagnosis of G. Vaginalis infection based on Amsel criteria and culture methods, the patients were randomly assigned to receive either intraVaginal 250 mg vitamin C (intervention group, n=24) or 250 mg metronidazole tablets (control group, n=24) for eight nights. The patients were evaluated 10 ± 2 and 30 ± 2 days before and after the treatment. Results: Culture and Amsel criteria showed significant improvement in both groups during the first and fourth weeks of treatment. However, the intervention group that received vitamin C showed a more substantial improvement (P<0. 002, P<0. 001). IntraVaginal vitamin tablets were more effective than metronidazole tablets in treating G. Vaginalis infection. Furthermore, vitamin C tablets improved the abnormal growth of Vaginal microflora and reduced the abnormally high pH of the vagina. This treatment effectively prevented the recurrence of G. Vaginalis infection for one month. Conclusion: Vitamin C Vaginal tablets showed to be a promising alternative treatment for G. Vaginalis infection, but further research is needed to confirm these findings and address potential limitations.

Abstract: 

Introduction: Cesarean is the most important and costly gynecology surgery. infection is the second important complication after hemorrhage. It seems that almost all risk factors of postpartum infection are due to ascending organism from lower genital tract to upper genital tract. Surgical technique is also an independent risk factor. According to the importance of the post partum infection in Iran and controversy in literature we compare the preparing of vagina before surgery as a method for organism reduction with routine cesarean section procedure.Methods: during a randomized clinical trial study patients came for c/s were conducted into two groups: 1-routine c/s, 2-Vaginal preparing before c/s.demografic data were collected, They were visited on the first post operative day, one week and six week after discharge and any fever, endometritis, febrile morbidity and wound infection were recorded for analyzing.Results: There was significant difference between the groups for the first day, first week and sixth week fever and febrile morbidity. But there was no significant difference was observed for first day wound infection, first day first week and sixth week endometritis. The frequency of those criteria which was mentioned; studied in patients with rupture of membrane and labor separately. There were no statistically significant deference between subgroups (those who had trial of labor and rupture of membrane).Conclusion: This study confirms the hypothesis that Vaginal preparing reduces the infectious rate. This simple method can use easily before surgery to reduce infection significantly.

Abstract: 

Background: The premature rupture of amniotic membrane (PROM) is considered as a cause of 1/3 of premature deliveries and has a reasonable effect on mortality and morbidity of newborns. So, on time and accurate diagnosis of PROM is necessary for keeping the safety of mothers and their fetuses. In this study sensitivity and specificity of Padtan-Pasargad kit in diagnosis of PROM were investigated.Methods: TThis diagnostic study was conducted on 30 pregnant women with premature amniotic sac rupture (case group) and 30 pregnant women without amniotic sac rupture (control group), who were been referred to Isfahan Clinic Hospital. The case and control groups were randomly selected. Vaginal Physical examination, Nitrazin test and Padtan-Pasargad test were performed for all cases.Findings: Based on the physical examination as gold standard, the sensitivity, specificity of Nitrazin test were 76.6 and 83.3% respectively. Sensitivity, specificity and accuracy of Padtan Pasargad test were 83.3, 93.3 and 88.3% respectively.Conclusion: The results of this work showed that Padtan Pasargad Vaginal pH &PROM kit have suitable sensitivity and specificity for diagnosing PROM and could be use as a rapid test in women with suspected   premature rupture of amniotic membrane.
